Why does the watch not reflect the updated settings made on the Noise Amigo app? 

Noise Explorer smartwatch takes a few minutes to reflect the changes made in the Noise Amigo app. 

However, if the watch does not reflect the changes then check the network connection, credit balance and the active data connection of the SIM.
